The client enrolment form will need to be completed and emailed to sally@pilateswithsally.co.uk prior to starting a class.

The form covers personal details such as your name and address and emergency contact information. It also contains a questionnaire covering your background and health, a section on your aims and a section to meet the new General Data Protection Regulation (GDPR) which comes into effect in May 2018.
